MAC Service awards
To simplify the submission for the MAC County Service Awards, we created an electronic form that can be filled out by the county clerk, staff of the county clerk, county auditor or your county’s HR officer, which can then be saved and emailed back to MAC.
The County Service Award is a Certificate of Merit for all county-elected officials and county employees who are celebrating the milestone of serving 5, 10, 15, 20, 25, 30, 35 and etc., years in 2024. In order for your county officials or employees to receive a certificate, please complete the form with the names of the officials and employees that have achieved one of the milestones. We ask that you fill out the text at the top of the form that includes: County name, Submitted by, Department, Date and Mail to (if the certificates need to be mailed out).
After completing that section of the form, begin typing in the names and years of service in the table. Please DO NOT USE ALL CAPITAL LETTERS and list the recipient of the award by their first name followed by their last name in the name column (example: John Doe).
When you’re done filling out the form, save the Excel file as “MAC County Service Awards - 2024 - (insert county).” Then email the Excel file back to David Owen at dowen@mocounties.com before the close of business on Friday, Sept. 27, 2024.
The awards can be picked up at the Annual Conference in November. Awards that are not picked up at the conference will be mailed out to the person listed on the form. If you know your certificates will not be picked up at the Annual Conference, please indicate that in the return email so we can have those ready to be mailed out.
